A deceptively difficult drill to load the long posterior kinetic chain, with contact through the foot for increased cerebellar engagement. Our cerebellum regulates the control of our limbs via mechanoreceptor feedback, and so the more parts of a limb you engage, the more attention it has to pay. Pressures is directed downward through the foot, pelvis is slightly lifted off the floor while lower back stays in contact to allow the limb to accept most of the load. This is layered with a VORc to engage the vestibular system throughout, as the vestibular system accounts for a huge amount of the activation we express in our posterior chain in upright movement. Our vestibulospinal reflexes help to set the amount of tone in our posterior chain based on our head movement and the functional demand of the task. Maximal intention is driven through the eyes, maintaining clear focus on my thumb with the movement. With movement based rehab, our goal is to allow for self-organization towards a movement that we express most as humans, which is gait. You don’t push off your heels to move forward, so the goal should be to allow for load acceptance through the foot. To express effortless movement, we must ensure quality engagement of the proprioceptive system, vestibular, and visual system in specific ways based on a good assessment. Systolic hypertension and an atlas (C1) adjustment (observation) A week or two a ago I experienced something interesting happen to another interns patient. This patient won’t be named for HIPPA purposes so let’s call him Mr. X. So Mr. X presented to clinic with decently high systolic blood pressure for his age. Systole is the top number on blood pressure which represents the amount of pressure exerted from the heart during contraction. He’s had this for a while. Recently the Normal ranges of blood pressure have been dropped to 120/80 from 130/90. Slightly drastic in my opinion because not very many people read below 120/80 when their blood pressure is being taken, but anyway. Mr. X was decently healthy and exercises / runs. The intern provided some research on potentially cutting certain processed carbohydrates and keeping a log of high sodium foods (since the DASH diet is the number one recommended/ research based diet for hypertension). We decided it would be interesting to have me adjust his atlas on the right. After motion palpation (one way a chiropractor finds a restriction/ decrease in mobilization) I applied a HVLA (high velocity low amplitude) adjustment and retook his BP. We took it moments before and moments after. His systolic BP dropped 25 plus points, remarkable. Obviously atlas has more mechanoreceptors than any joint in the body (Cocciput to C1). Very close to the feet, research goes back and forth on this. It’s closes to the brain stem and it’s where the vertebral artery makes a very sharp turn (in a very smooth vertebral foramen) and eventually joins to make the basilar artery. Honestly, there could be many reasons why this happened and I’m going to make no assumptions. I could think of 5 quick reasons why it could of happened! Just wanted to share this! One quick theory is the up regulation of the parasympathetic nervous system or because C1 is so close to the vagal nucleus in the medulla! Ezra came with me to physical therapy today and she did so well!! I was very nervous to ask if I could bring her for training because the last time she went to a doctors office she was very anxious and broke her downstay several times. She seemed very concerned by people touching and examining me. We’ve been practicing a lot since then, and today she placed on her mat and held her downstay the whole time! She watched me have blading done, laser, eSTEM, and stayed as I moved around to do my exercises. She wore her ecollar but I never had to use it! I plan on taking her with me to my doctor’s appointments in the future. Big thanks to our awesome PT team!
